To book your pet's Animal Health Certificate collection we require this application form and your pets rabies vaccine details

By submitting this application form... 🐾 You declare that the information entered is accurate and correct and can be used by an Official Veterinarian to complete an Animal Health Certificate 🐾You understand that you will be asked to sign a declaration confirming that the pet(s) are not travelling for sale or to transfer ownership and the pet(s) will travel within five days of the owner/authorised person.

🐾You are aware that the certificate must be collected in person by the owner named on the certificate and the pet(s) must be brought to collect the certificate so their microchip(s) can be scanned

Pet Travel Pass is a mobile veterinary certification service; we do not have a shop or clinic. This keeps our overheads to a minimum so we can pass the savings on to pet owners. We collate all the information we need to make your pet's AHC online, then meet up with you and your pet in the Nottingham area to scan their microchip and issue the certificate. We meet people at either Sherwood Business Park NG15 0EA or Giltbrook Retail Park NG16 2RP; the location depends on the date of your collection. You can find our prices here.
